SASI

What is it?

The Single-Anastomosis Sleeve-Ileal (SASI) procedure combines a sleeve gastrectomy with one connection between the stomach and the distal small intestine (≈ 2.5–3 m from the colon).

Who are suitable candidates?

  • BMI ≥ 35, regardless of associated health problems.
  • Repeated unsuccessful weight-loss attempts.
  • Presence of reflux symptoms, where a bypass component may help.

ADVANTAGES

  • Durable weight loss: Usually 25–30% TWL.
  • Hormonal appetite suppression: Via GLP-1, PYY, and reduced ghrelin.
  • High metabolic effect: Improves or resolves diabetes in up to 60–80% of patients.
  • Improvement of associated conditions: Hypertension, dyslipidemia, sleep apnea, fatty liver, joint disease.
  • Reduced reflux pressure: May improve reflux in selected patients, though bile reflux can occur—careful selection is key.
  • Low deficiency risk: As part of food still travels the full intestinal length.
  • Faster recovery and sustained results.

THE PROCEDURE

  • Surgery duration: 60–80 min
  • Hospital stay: 2–3 days
  • Recovery time: ≈ 2 weeks
  • Expected weight loss: 25–30% TWL
