One-Anastomosis Gastric Bypass

What is One-Anastomosis Gastric Bypass (OAGB/MGB)?

The one-anastomosis (mini) gastric bypass creates a small stomach pouch connected to the small intestine, bypassing a segment to limit both food intake and calorie absorption.

Who are suitable candidates?

  • BMI ≥ 35, regardless of associated health problems.
  • Failed non-surgical weight-loss efforts.
  • Fully informed and motivated adults (18–65 years; individualized exceptions).

ADVANTAGES

  • Significant weight loss: Typically 25–35% TWL.
  • Improvement or remission of type 2 diabetes, hypertension, sleep apnea, and cardiovascular risk.
    • In the YOMEGA study, OAGB achieved superior long-term diabetes control compared with Roux-en-Y gastric bypass (60% vs 38%).
  • Simpler technique: Only one intestinal connection, shorter operative time.
  • Appetite control: Incretin hormones (GLP-1, PYY) enhance satiety.
  • Risks to monitor: Bile reflux and nutritional deficiencies remain possible—lifelong supplements and surveillance are essential.

THE PROCEDURE

  • Surgery duration: 60–80 min
  • Hospital stay: 2–3 days
  • Recovery time: ≈ 2 weeks
  • Expected weight loss: 25–35% TWL
